Value-based care provider Monogram Health is teaming with Memorial Hermann Health Plan to provide in-home services to chronically ill patients covered under the insurer’s commercial and Medicare Advantage plans.
Under the joint-venture agreement, Monogram Health will provide home-based care to patients in the Houston area with complex conditions, including kidney disease, diabetes, high blood pressure and metabolic disorders, the two organizations said Tuesday in a news release.

Monogram assists patients with medication and symptom management, as well as helping them address social determinants of health.
Memorial Hermann Health Plan is the insurance division of Houston-headquartered Memorial Hermann Health System, whose more than 6,000 physicians will help coordinate home-based services with Monogram, according to the release. The health system's insurance plans cover approximately 50,000 members.
Value-based care providers such as Monogram, WellBe Senior Medical and HarmonyCares have increasingly collaborated with health insurers to try and keep medically complex patients out of the hospital.
Monogram is backed by Frist Cressey Ventures, Norwest Venture Partners, TPG Capital and other investors. It has alliances with risk-bearing providers and health insurers, covering approximately 200,000 lives across 36 states.
Memorial Hermann owns and operates 14 hospitals and 270 care delivery sites across the Houston area.
