    Craig Bolda, MSPT, MBA

    Community Healthcare System

    Craig Bolda has worked in Northwest Indiana with Community Healthcare System (CHS) for almost 30 years. Mr. Bolda began his career with CHS as a staff physical therapist at Community Hospital and later served as CHS Regional Director of Rehabilitation and Therapy Services for more than a decade. He has experience implementing new healthcare delivery systems in detailed and complex regulatory environments. Mr. Bolda also has experience in leading work teams, managing operating budgets, and overseeing facility operations. He served as Chief Operating Officer at St. Catherine Hospital, part of the CHS, from 2010 to 2017. Mr. Bolda currently uses his expertise and experience in addressing the healthcare needs of Northwest Indiana and Chicagoland in his role as Vice President of Acute Rehabilitation Services for Community Healthcare System. He holds a Master of Science degree in Physical Therapy from Finch University of Health Sciences at the Chicago Medical School and a Masters of Business Administration degree from Indiana University. 
     

    Sally Brooks, MD
    Chief Medical Officer
    Kindred Healthcare

    With over 25 years of leadership experience, Dr. Brooks fosters partnerships with physicians, healthcare systems, insurance organizations and professional societies. She was appointed CMO of Kindred Rehabilitation Services in January 2015. In her role, she collaborates with medical directors and leading experts to innovate and leverage best practices across multiple sites of care, advocating safe and effective care transitions and high-quality results. Dr. Brooks holds degrees in pharmacy and biology in addition to her medical degree. She is a board-certified internist and geriatrician and Fellow of the American College of Physicians and American Geriatrics Society. She is a member of AMRPA, advocating for acute inpatient rehabilitation services.
     

    Cathy Knight
    Division Vice President
    Kindred Healthcare

    Cathy Knight is a Division Vice President with Kindred Healthcare.  She is a Registered Nurse with over 25 years of clinical and leadership experience in the field of rehabilitation, both in Canada and the US.  She is a graduate of Queen’s University, Ontario, Canada, and is currently in the graduate program, nursing leadership at the University of Kansas.

    Cathy was the Vice President of Patient Care Services for St. Elizabeth Health Care, in Toronto, Canada, as well as the Chief Operating Officer of a large rehabilitation hospital in Kansas City. She was a founding board member of the American Stroke Foundation in Kansas City, and the president and board member of Independence Inc., a community based independent living center serving north east Kansas.  She is currently the president of Mufindi Orphans Inc., an NGO serving vulnerable children and families in rural Tanzania.
