Readmissions penalties are assigned by CMS Certification Number, so in hospital groups that share a single number, only the principal hospital is named. Data assembled and analyzed by Modern Healthcare using 2013 and 2017 CMS Readmissions Adjustment Factors, which are based on 30-day readmissions rates for major conditions, for fiscal 2013-17.
