Mihaljevic has led the renowned multispecialty academic health center since 2018. In his annual State of the Clinic address in January, Mihaljevic reported the system provided almost 14 million patient encounters in 2023 — the most in its history. It now provides care at nearly 300 locations on three continents. The organization also allocates more than $500 million annually to research, innovation and education. The main campus added 45,000 square feet of renovated lab space last year and will be adding two new research buildings. In October, Cleveland Clinic announced a collaboration with Amazon One Medical, a hybrid virtual and in-person primary care provider. Mihaljevic joined Cleveland Clinic in 2004 as a cardiothoracic surgeon and has held several other leadership roles, including two years as CEO of Cleveland Clinic Abu Dhabi.
