At Queens, New York-based MediSys, Raoof has led her team in gaining deeper insights into the organization’s diverse patient population, who speak more than 135 languages and who are mostly covered by Medicare and Medicaid. She convened a committee of nurses, physicians, case management teams and information technology leaders to develop a workflow to collect data on health-related social needs of patients and trigger automatic referrals through the electronic health record. The collected insights helped increase community uptake of COVID-19 vaccines and boosted MediSys’ Patient Navigation Program, which in turn led to reduced colonoscopy no-shows.
