After several years advocating on behalf of vulnerable individuals requiring complex care, Moyer joined the Boston-based integrated care system in 2021. Since then, she has helped foster CCA’s hospital-to-home program, which works with four high-volume hospitals to assess CCA members for unmet medical, behavioral and social needs. Moyer also led the development of a new health and functional assessment to better determine each patient’s eligibility for 14 different services. The organization now uses the tool immediately upon member enrollment.
