CarePort, which is now owned by WellSky, aims to coordinate data as patients transition from acute to post-acute settings, as well as through accountable care environments. The technology is used by more than 1,000 hospitals and 110,000 post-acute providers and the company says its platform handles nearly 40% of hospital discharges to a post-acute provider.
