Charbonneau was a key part of Encompass’ efforts to launch an innovative data-mining tool that analyzed information in the system’s electronic health record system in order to target interventions for patients who were at high risk of a readmission. The initiative has resulted in more patients being discharged safely to their communities and fewer transfers to acute-care hospitals.
