Charbonneau was a key part of Encompass’ efforts to launch an innovative data-mining tool that analyzed information in the system’s electronic health record system in order to target interventions for patients who were at high risk of a readmission. The initiative has resulted in more patients being discharged safely to their communities and fewer transfers to acute-care hospitals. She also serves as faculty for development of a continuing education program on stroke care, which is housed on the American Heart Association’s website. Charbonneau is also active on the policy front, lobbying on behalf of rehab facilities and being named to the National Quality Forum’s measurement panel for post-acute care.
