Sieboldt has homed in on reducing gaps in care for vulnerable populations, including in skilled-nursing facilities. She addressed social determinants of health that affect senior care such as food insecurities, access to care and social isolation as a component of mental health. She also worked with SNF organizations to examine problems that led to readmissions and to figure out ways to improve palliative care. Sieboldt also helped Deaconess’ Next Generation ACO achieve savings of $17 million and $541 per Medicare beneficiary.
