Under Swarthout’s leadership, the pharmacy team at Johns Hopkins developed a program to better coordinate prescriptions for patients prior to discharge as well as a post-discharge pharmacist home visit program. The programs have resulted in a 54% drop in readmissions caused by medications. Her team is now working on improving care transitions with skilled-nursing facilities.
