Izquierdo-Hernandez focused recently on increasing home health services at UHPs network of 10 community health centers in New York City. The effort aims to better coordinate care for patients with chronic conditions, including mental health and substance abuse, especially frequent emergency department users. The initiative helped increase UHPs revenue 38% over 2016. Izquierdo-Hernandez also increased UHPs reliance on value-based payments by 188%. Clinically, UHP now exceeds objectives in two areas for HHS Healthy People 2020 initiativebirth weight for infants and controlling high pressure. Also on the population health front, Izquierdo-Hernandez created a public health council that aims to tackle such topics as healthy eating.
