Ms. Hodgman is responsible for designing and implementing new models of care coordination designed to improve care management and care transitions across settings that seek to improve patient outcomes and experience while transitioning patients efficiently and seamlessly through the post-acute continuum.

Beth Weagraff serves as AHS's vice president of post-acute care development and implementation. In this capacity, she is responsible for the development of the AHS Post-Acute strategies and ensuring successful deployment of various post-acute initiatives. Prior to this position, she served as the COO for Florida Hospital Medical Group and prior to that served as senior vice president for Florida Hospital, where she provided oversight for the Cardiovascular Institute, Neuroscience Institute and the Institute for Surgical Advancement.
