This contest is open to all new registrants to Modern Healthcare who are 18 years or older and legal residents of the United States. Employees of Crain Communications, Inc. and affiliates, its advertising, promotion, marketing and production agencies and immediate members of their households are not eligible.

1. You will be automatically entered when you complete the Help Us Help You – Subscription Feedback survey at https://www.surveymonkey.com/r/VQNKZND.
2. To enter without registering and with no purchase necessary, send an email to [email protected] with “Enter the Modern Healthcare Help Us Help You Subscription Feedback Survey Drawing” on the Subject Line and include your full name, email address, and telephone number with area code in the body of the message. Entries will not be acknowledged or returned.
3. Only one entry per person permitted.

All registrations and e-mail entries must be received on or before the last day of each calendar month throughout the duration of the contest. Crain Communications Inc has no responsibility for lost, late or misdirected e-mail messages. Prize winners will be determined once per month at a random drawing of eligible entries on or before the 15th of each calendar month throughout the duration of the contest at Crain Communications Inc, 150 N. Michigan Avenue, Chicago, IL 60601. The winner will be notified by e-mail or telephone within 3 business days of the random drawing. The judges' decisions are final and binding.

Each month throughout the duration of the contest, one (1) Amazon gift card with a value of approximately $25 each. No substitution or transfer of prize is permitted. The odds of winning depend upon the total number of eligible responses received.
