Primary-care networks Tandigm Health and Innovative Wellness Alliance are entering into an agreement with Huntingdon Valley, Pa.-based Holy Redeemer Health System that will bring the organizations closer to a population-health model.
The cooperative arrangement will bring in physicians from Tandigm and Innovative Wellness to practice in Holy Redeemer's acute-care hospital and provide care in outpatient services, home health and long-term-care facilities.
Tandigm is a West Conshohocken, Pa.-based integrated-delivery system jointly owned by DaVita HealthCare Partners and Independence Blue Cross. Innovative Wellness is a clinically integrated network owned by Holy Redeemer and its practicing physicians, based in Huntingdon Valley, Pa.
The three organizations will share patient-management risk as a part of the agreement, effective March 1. No formal joint venture or merger is involved, and governance structures remain the same for all of the organizations.
