    Clinical Integration: What Really Matters

    As value based delivery and financing increasingly becomes reality, successful organizations will adapt their delivery framework to adjust from pure fee for service to performance based reimbursement, with reductions in the historical rates of increases. As a result, we are at the point where we must re-engineer how care is delivered. Doing the things that achieve the long-hanging fruit will not be enough.

    That is where Clinical Integration becomes essential. The only way to reengineer care is in partnership with your physicians- indeed, it is only through this partnership that you will be successful in the next era of healthcare reform.

    There are 5 things we must do very well to be successful:

    1. We must truly and deeply engage with our physicians around quality. To do so requires new partnership models. New organizational structures with shared governance and physician leadership must be established. Physicians participating must understand the WIIFM- “What's In It For Me?”, which means there must be a clear value proposition for their involvement. This is nothing less than a mission change for most hospitals.
    2. We must invest in new infrastructure for care delivery. This means Electronic Medical Records, but it also means predictive analytic capabilities, a commitment to and a way to deliver Evidence-Based Medicine to participating physicians, support of the Patient Centered Medical Home, and a new breed of healthcare professionals who can team to manage care across the continuum.
    3. We must adapt to lower reimbursement and take costs out of the system, particularly for inpatient stays. This means a commitment to continual improvement in LOS and resource consumption.
    4. We must fine-tune our operations for maximal efficiency. We have to eliminate waste in our processes and be able to do more with less resources.
    5. We have to be able to take and manage financial risk. We need new competencies to assess risk, understand and manage population health dynamics and negotiate with payers around both performance based rewards and even capitated payments.

    Dr. Steven T. Hester was promoted to senior vice president & chief medical officer in September 2008 after serving as vice president, medical affairs, since December 2006. He joined Norton Healthcare in January 2005 as chief medical information officer. Prior to that Dr. Hester practiced Emergency Medicine full time, serving as regional medical director for Kentuckiana Emergency Physicians. During his tenure there he worked to improve quality and volume with significant success, while redefining multiple processes within the organization. He has served Norton Healthcare in multiple medical staff positions, including president of the medical staff for a Norton Healthcare facility and a representative on the System Medical Executive Committee. Dr. Hester holds bachelor's and medical degrees from the University of Louisville and is board certified by both the American Board of Internal Medicine and the American Board of Pediatrics. He holds a master of business administration degree from Bellarmine University. Dr. Hester serves on the board of directors of Kentucky Harvest, City on a Hill Productions, and Community Advisory Committee for a Foundation for a Healthy Kentucky. Dr. Hester serves on the Medical Advisory Board for West Wireless Health Institute. He is a member of the American College of Physician Executives, American Medical Association, Kentucky Medical Association.

    Gary Smalto, M.D., MBA

    Managing Director

    Dr. Gary Smalto is a Managing Director in the healthcare group of FTI's Corporate Finance practice and is based in Washington, DC. Dr. Smalto is a seasoned physician executive with over 20 years of experience in clinical practice, hospital operations, and change management within hospital systems. He has been instrumental in designing and implementing cutting-edge initiatives

    around the establishment of Accountable Care Organizations, and has implemented clinical operational improvement programs in over 30 hospitals, involving over 150 staff.     Gerry Meklaus

    Senior Managing Director

    Gerry Meklaus is a senior managing director in the healthcare group of FTI's Corporate Finance, where he leads the Physician Enterprise consulting practice. He has over 12 years of consulting experience and 25 years of diversified healthcare management profit and loss experience. As a consultant, he has led the delivery of highly successful projects for large group practices, prominent faculty practice plans, academic medical centers, community hospitals and for-profit entities. As an executive, he has overseen the successful design and development of new ventures, from integrating acquired medical groups, through new ambulatory facility development, through the introduction of new medical devices to US users. Mr. Meklaus is recognized as a thought leader in the area of physician-hospital affiliation strategies and business arrangements. He has assisted health systems with a variety of strategic and tactical challenges in this area, from integrating and optimizing medical group acquisitions to the development of Accountable Care Organizations. Mr. Meklaus has also functioned in interim leadership roles for organizations undergoing change. He has authored thought capital and spoken on topics related to optimizing physician-related transactions and business arrangements. He is regarded as a pro-active change agent who works closely with clients to ensure that results are sustained well after the consulting engagement ends.

    Tad Schweikert

    Tad Schweikert is a senior managing director in the healthcare group of FTI's Corporate Finance. Mr. Schweikert has more than 15 years of healthcare experience providing operational, financial and strategy advisory services to healthcare providers, payers and technology solution vendors. Mr. Schweikert has significant healthcare expertise in supply chain management, revenue cycle management, business planning and process redesign.
