I have worked in private academic organizations, university medical centers and community-based organizations, and one of the key quality issues relates to how we pass off the patient from a care perspective. As specialists and subspecialists have become more prolific (and rightfully so), this issue has only gotten worse ... and the problem is simply one of communication and a matter of professional courtesy. Everyone is so busy that the process gets pushed way down on the to-do list. It's not rocket science. It's a communications problem.
