Scribes, ancillary staff can complete certain forms
The only elements of the history that Medicare allows anyone (in addition to the physician) to obtain and document are the review of systems and the past medical, family and social history. To acknowledge that the physician has reviewed the information recorded by ancillary staff or on a form completed by the patient, the physician must sign and date the form.
