In separate notices, two state insurance departments announced fines against units of Minnetonka, Minn.-based UnitedHealth Group for a range of violations. The Texas Insurance Department announced a mid-August order against United Healthcare Insurance Co. and United Healthcare of Texas, which fined the companies $3 million for failure to comply with utilization-review procedures and regulations; failure to obtain review of advertising by the department; failure to pay clean claims penalties; and for improper discounting of fees for noncontracted providers. Kim Whitaker, a spokeswoman for United Healthcare in Plano, Texas, said the companies have paid $1.1 million to providers, and the remainder is pending to the department. The Colorado Division of Insurance separately said it imposed a fine of $211,000 against United Healthcare of Colorado and $246,000 for United Healthcare Insurance Co. for a total of $457,000.
