Bridges to Excellence is launching five new physician recognition programs to reward improvements in chronic-care management.
The not-for-profit quality improvement group will provide incentives to physicians in the following care areas: asthma, congestive heart failure, chronic obstructive pulmonary disease, coronary artery disease care and hypertension.
Physicians who demonstrate excellence in care and meet key benchmarks will receive bonuses per patient per year for each patient whose employer or health plan participates in the program.
Our work has shown that there are significant avoidable costs associated with the management of these conditions, said Francois de Brantes, CEO of Bridges to Excellence. By reducing preventable complications incurred by most patients with these chronic conditions, we could significantly improve the health of employees and save employers billions of dollars.
