If you look at the 10-year cost of acquisition and maintenance of an electronic health-record system, most of the costs aren't in software licensing but in hardware and software updates and support expenses. The VistA system and its Mumps/Cache-based data structure are support nightmares in today's MS operating systems and relational database worlds. The support sources for VistA-derived EHR products are limited and expensive compared with the commercial vendor productswhich themselves are priced beyond what the current practice marketplace will support. And the VistA-based products themselves are rigid and unattractive compared with the commercial vendor offerings. At least that has been our experience.
