Computer disks containing the Social Security
numbers and other personal information of about 1,400 veterans
treated at a McAlester clinic disappeared, federal officials said.
The Veterans Affairs Hospital in Muskogee confirmed the loss in a
letter mailed Tuesday to patients of the McAlester clinic. The lost information includes patients' names, Social Security
numbers, amounts billed and amounts paid for services they received at
the outpatient clinic, which is contracted through the Choctaw
Three disks had been mailed separately from the hospital to the
clinic, hospital spokeswoman Nita McClellan said.
The lost information was mailed on May 10, June 10 and July 10,
McClellan said. She said the delay in notifying veterans was caused by having to wait on federal officials in Washington to approve the form letter. That approval didn't come until Oct. 26, McClellan said.
