The CMS said it will choose 10 initiatives, each serving 15,000 to 30,000 Medicare beneficiaries, for a disease management program mandated by the new Medicare reform law. The three-year program aims to show that helping beneficiaries self-manage three chronic conditions -- congestive heart failure, complex diabetes and chronic obstructive pulmonary disease -- will improve care and reduce Medicare costs.
