The CMS announced a program under which it will match what states spend on disease-management programs for chronic conditions such as asthma, diabetes and congestive heart failure. A CMS letter to state Medicaid officials outlines several models for developing such programs, including contracting with outside organizations. The agency's announcement comes on top of two disease-management pilot projects authorized under the new Medicare law. One project will provide bonus payments to providers that adopt technology to improve the health of patients with chronic diseases, while the other will offer technical support to physicians and self-care information to patients. -- by Tony Fong
