The American Hospital Association recognized three hospitals and a healthcare system for leadership and innovation in creating a culture of safety -- a blame-free environment in which reporting of medical errors is encouraged and facilitated among staff, patients and families. The AHA awarded its 2003 American Hospital Quest for Quality Prize to 452-bed Abington (Pa.) Hospital, which will receive $75,000. Finalists were 967-bed William Beaumont Hospital, Royal Oak, Mich., and 471-bed University of Wisconsin Hospital and Clinics, Madison. A citation of merit was given to 203-bed Olympic Medical Center, Port Angeles, Wash. -- by John Morrissey
