In "Nowhere to go but up," part of your Jan. 3 cover story (p. 34), your readers saw an optimistic portrayal of Medicare's proposed prospective payment system regulations for home care. Many believe the proposed regulations contain understated rates, additional cash-flow delays and other provisions that pose a serious threat to the viability of both hospital-based and freestanding agencies.
HCFA has received more than 350 comments containing compelling justification for revisions. We believe it will be essential for many of these changes to be introduced into the final regulations to minimize devastating results once the PPS is implemented.
