CCF Health Care Ventures, a subsidiary of the Cleveland Clinic, has agreed to manage a new post-acute services program for the entire Cleveland Clinic Health System.
Terms were not disclosed.
The participating system members include the Cleveland Clinic Foundation, 428-bed Fairview General Hospital, 322-bed Lakewood (Ohio) Hospital, 174-bed Lutheran Medical Center, 215-bed Marymount Hospital in Garfield Heights, Ohio, and Meridia Health System of Mayfield Village, Ohio.
CCF will coordinate the healthcare providers' home-care, hospice, subacute-care, skilled-nursing, rehabilitation, assisted-living and therapy services.
CCF also will add disease management programs, patient outcome analysis, clinical guidelines, sales support and corporate compliance programs to the system's continuum of post-acute services.
The participating hospitals said the new arrangement will help them attract managed-care contracts, keep patients within their system and generate more revenues for system members. The program will be phased in over the next two years.
