Participants in the largest Medicare accountable care organization model are calling on the Centers for Medicare and Medicaid Services to extend the initiative after it produced record savings.
The ACO Realizing Equity, Access and Community Health, or ACO REACH, model collectively generated $1.6 billion in gross savings and $695 million in net savings in 2023, according to a report CMS published Friday. Nearly three-quarters of participants earned net savings.

But CMS hasn’t said whether ACO REACH will continue past its scheduled expiration in 2026, which is stoking anxiety among participants that have made up-front investments.
“Many of the things that we have done to provide better care require infrastructure, require support. When there's any uncertainty around what the future of that looks like in terms of how we're going to fund it, it’s worrisome for us," said Dr. Victoria DiGennaro, CEO of Uniontown, Ohio-based Pioneer Physician Network and a partner with Austin, Texas-based Agilon Health.
