The Commonwealth Fund/Modern Healthcare Opinion Leaders Survey, conducted by Harris Interactive, is an online poll designed to highlight perspectives on the most timely health-policy issues from a panel of the nation's leading health plan executives, policymakers and healthcare purchasers. It is conducted four times annually. This is the 22nd installment in the series.On pp. 24 and 25, perspective on the latest topic—delivery system innovation and improvement—is provided by HHS Secretary Kathleen Sebelius and former Sen. Ensuring success | Must focus on patients, eliminating inefficiencies

Our current healthcare system is full of competent and caring doctors, nurses and other healthcare professionals working hard to do right by their patients. But the system as a whole just isn't delivering what patients or providers need. Dysfunctional incentives create fragmentation and fail to address the underlying drivers of disease. In addition, far too many decisions are now in the hands of insurance companies, not doctors and patients.

Reason for optimism | Key roles for states, providers, insurers a good thing

As someone who has been doing health policy reform for 35 years at the local and national levels, I am beginning to get that Winston Churchill feeling—that Americans eventually get it right, but only after they've tried everything else. The process by which the Patient Protection and Affordable Care Act became law was quite partisan, but much of the new law is not partisan at all. In fact, I am amazed that, despite the process, Congress managed to finally get the policy goals right.

While I have profound disagreement with what President Barack Obama and the Democratic congressional leadership did with passage of the Patient Protection and Affordable Care Act, their moment of jubilance is understandable. It was a significant legislative accomplishment.
