A: There is no fee to participate, however a nominal fee is charged for the use of paper surveys.
Q: When is the deadline?
A: The entry deadline for 2013 was May 17, 2013.
Q: What type of questions are asked?
A: The program collects information from both the employer and employees and is open to all companies (providers, suppliers, payers) with at least 25 employees.
Employers complete a survey detailing:
