Adventist Health System's Readmission Reduction Through Glycemia Management Solution is a composite of items to help facilitate improved glucose management in the hospital inpatient environment. This Solution is currently in-use at Adventist Health System, including the world-recognized Florida Hospital.

The Solution materials can be used to educate providers and ancillary personnel involved in glycemic improvement efforts. Additionally, template ordersets provide a foundation for standardized approaches to glucose control.

Objectives

How are the components of The Readmission Reduction Through Glycemia Management Solution used?
The care sets and other materials provided in the Solution are used in the day-to-day management of patients. Additionally these materials were used at Adventist Health System for the creation of computerized physician order-entry (CPOE) ordersets that are embedded in electronic medical records (EMR) and used widely.

What problems or objectives does this Solution address?
The Readmission Reduction Through Glycemia Management Solution provides materials for the management of in-patient hyperglycemia using intravenous insulin therapy or subcutaneous multi-modal insulin approaches. Materials include: concise "How-to Paper" addressing most aspects of inpatient glucose management, key references supporting need for improved glucose control, power point presentations that are ready-to-use, and template ordersets that can be readily implemented.

How was the Solution developed?
The Glycemia Solution materials were created by Adventist Health System with input from a number of clinicians involved in diabetes and glycemic management from Adventist's large multi-institutional health care system. The materials have been utilized and modified by “real life experiences” at Adventist's health care facilities.

Implementation

Were there any organizational considerations?
For the past several years, Adventist has had a strategic initiative to improve glycemic care in their facilities. These Solution materials have provided much of the foundation for that endeavor.

Which Benchmark/Pertinent Metrics are recommended to use with this Solution?
Adventist closely monitored both the ICU and Non-ICU Settings glucose metrics (such as average glucose, hypoglycemia rates, multi-modal insulin usage versus sliding scale insulin, etc.)

Who was involved in the creation of this Solution?
The creation of the Solution required input from endocrinologists, certified diabetic educators, intensive care providers and other clinicians with an interest in diabetes/glycemic management.

Any advice to other hospitals and health systems in preparation to launching this Solution?
The identification of Clinicians to champion a glycemic project is essential. The materials such as ordersets should be tested on a smaller scale and modified based on the pilot experience prior to any system-wide application.

This Solution has been known to improve average glucose levels and hypoglycemia rates. In order to accurately monitor success, be sure to measure:
- Days in ICU
- Overall Length of Stay
- Morbidity
- Cost
- Compliance Rates (% of time it's being used accurately)
- Percentage of Use (% of staff using it)
