Modern Healthcare is conducting its 36th annual survey of multi-hospital systems. This survey is intended for U.S. organizations that own, lease or sponsor two or more acute-care or psychiatric hospitals with a unique Medicare provider number.

REQUIRED: A senior executive from the company must review the completed survey before submission then sign this document to verify the accuracy of data.
Completed surveys are due
March 30 by 5 p.m. Central, with selected results to appear in the June 4 issue of
Modern Healthcare.
