About the Survey

This survey provides essential information on hospitals systems. Organizations are asked to send in their IRS Form 990s for every hospital or stand-alone entity within the system, or SEC Form 10-K for publicly traded organizations. This collection request is new as of 2013. This survey was first published in 1976.

Q: How can my system participate?

A: To participate, fill out the survey form on behalf of your organization and turn it in before the stated deadline. The survey form will be available here once it is open for participation. You may add your information to our notification database so that you can receive an e-mail directly once the survey is open for participation.

Q: What else do I need to do to participate?

A: Please download a copy of the Hospitals Systems Survey's survey definitions. REQUIRED: A senior executive from the company must review the completed survey before submission then sign this document to verify the accuracy of data. Once the document has been signed, it must be sent to the Research Department either via fax to 312-280-3159 or e-mailed to research@modernhealthcare.com.

Q: When will the survey open?

A: The 2013 Hospital Systems Survey is scheduled to open Feb. 25.

Q: When is the completed survey form due?

A: The 2013 Hospital Systems Survey has an initial deadline of March 25 for the first component (submitting the filings) and the questionnaire component is due April 26.

Q: What happens with this survey's results?

A: The data collected from this survey goes to create charts and rankings that illustrate a Special Feature story that is written by a Modern Healthcare reporter. Visit our Data Center for the most recent Hospital Systems Survey results.

Q: When will the results be published?

A: The 2013 Hospital Systems Survey results are slated to appear in the June 24 issue of Modern Healthcare.

Q: How much does it cost to participate?

A: There is no cost to participate in this survey.

Q: Can we still send in information if the deadline has passed?

A: Yes, the link will still be active for you to submit online. However, your information will not be taken into consideration for the survey results. A late submission will not be considered for participation. It will, however, be saved into the database so that it can be accessed for next year's survey.
