The National Transitions of Care Coalition has formed a work group to focus on health information technology.
The work group will assess barriers and gaps related to transitions of care and focus on how national efforts to develop electronic health records, information exchange and interoperable technology lead to improved patient care, the coalition said in a news release. The work group expects to develop a white paper with its recommendations.
Among the work group members are Leah Binder, CEO of the Leapfrog Group; David Dorr, assistant professor at Oregon Health & Science University; and Jean Paul Gagnon, senior director of U.S. policy and strategic advocacy for drug developer Sanofi-aventis U.S.
The coalition is coordinated by the not-for-profit Case Management Society of America, which advocates for the case-management profession, and is funded in part by Sanofi-Aventis U.S.
