TransforMED, a wholly owned, for-profit subsidiary of the American Academy of Family Physicians, has developed a networking Web site for helping primary-care practices implement the medical home model of care.

Called Delta-Exchange, the site will provide case studies, how-to articles and other resources that provide information on topics such as developing team-based care, maximizing office space and managing change, a news release said.

Users will be charged a $30 monthly fee and will be able to post documents, share images and videos and create wiki pages. The functions of Delta-Exchange are said to be based on the findings of a two-year, medical-home national demonstration project.

"The NDP taught us that many primary-care practices feel isolated, and many practice leaders are reluctant or unable to hire a consultant to help them transform to a patient-centered medical home," said Terry McGeeney, president and CEO of TransforMED, in a news release.

McGeeney said during a webinar that a "soft launch" took place more than a month ago so that the site would already would be populated when it opened for public participation today. About 250 "thought leaders" were part of this initial effort, including all of the participants in the medical home national demonstration project.

During the webinar, TransforMED marketing coordinator Nathan Bieck said Delta-Exchange would have no vendors, no drug reps, no advertising and no data mining.
