States should consider five major strategies when implementing medical home projects, according to a new report on the topic
Looking at 10 states with well-developed medical home programs, researchers said states should: form partnerships with key players; define “medical home” to set expectations; align reimbursement to reward practices; provide support to physician practices; and measure results.
The National Academy for State Health Policy based these recommendations on Medicaid and Children’s Health Insurance Program medical home initiatives in Colorado, Idaho, Louisiana, Minnesota, New Hampshire, Oklahoma, Oregon, Washington state, North Carolina and Rhode Island.
“State Medicaid and CHIP have strong foundational bases to develop policies that support medical homes,” the academy said in the report, which was supported by the Commonwealth Fund.
