MemorialCare Health System, a non-profit six hospital system located in Southern California, is currently seeking a Vice President of Population Health Management. The VP, Population Health, will oversee the development, integration and deployment of MemorialCares Population Health strategic roadmap in pursuit of the Triple Aim improving the health of the populations we serve, improving the experience and quality of care, and decreasing per capita cost for patients and their families. Reporting directly to our SVP for Performance Improvement, this individual will work with system-wide senior leadership, physician practices and the community, in order to spearhead our efforts to strategically move away from an episodic, illness based model and towards a model of managing health across populations.
The Vice President of Population Health will oversee a system-wide planning and execution team which is organized into ten work streams to address Care Coordination, Prevention, Technology, Vertical Integration, Workforce, Engaging People, Leadership, Community Resources, Innovation and Marketing. Specific linkages include facilitating participation in related demonstration projects (e.g. bundled payment, working with payor-based ACO teams), coordinating with campus and regional vertical integration planning, collaborating with end users and Information Systems team to ensure the design and creation of effective, efficient and user-friendly tools and technologies to support population health, and identify technology enhancements and opportunities not yet fully explored or evaluated.
Additional focus areas include: networking and building internal, community and regional relationships, working with representation from every campus to leverage clinical and operational expertise across the continuum of care; collaborating with our Physician Society and Performance Improvement initiatives to maximize VBP/Pay for Performance opportunities related to population health opportunities; contributing to MemorialCares external advocacy efforts for population health; and identifying and managing external consultative resources to support the work.Minimum Position Requirements:
This is an executive level position requiring 10 years health care leadership including in an executive level role, with experience in analytical modeling and complex strategic deployment. Continuum of care services experience preferred. Must have a Masters or Doctorate/Doctor clinical degree and education faculty experience.
